TARGET AUDIENCE

This activity is intended to meet the needs of physicians, nurses, nurse practitioners, pharmacists, physician assistants, psychologists, social workers and other healthcare providers who participate in end-of-life patient care and family support.

LEARNING OBJECTIVES

Upon completion of the educational activity, the learner should be able to:

  • Discuss the case management standards of practice and their application to end-of-life issues;
  • Identify the principles of pain management, palliative care, and symptom management near the end of life;
  • Review the concepts of death, loss, grief, bereavement, and coping strategies for patients and families;
  • Apply the four levels of hospice care and the new Medicare hospice consultation rule.

Method of Participation

In order to receive credit, participants should read the entire monograph, complete and submit an evaluation and score at least 70% on the self-assessment test.
