Josh Rajkumar, PharmD, BCPS is a clinical pharmacist specializing in advanced heart failure and cardiometabolic disease at Ascension St. Vincent Heart Center in Indianapolis, Indiana. He practices as an embedded ambulatory care pharmacist within the Advanced Heart Failure and Transplant Cardiology program, where he leads medication optimization for patients with heart failure, obesity, and obstructive hypertrophic cardiomyopathy.

Dr. Rajkumar has developed innovative pharmacist-led clinical programs focused on rapid implementation of guideline-directed medical therapy, GLP-1 receptor agonist initiation and titration, and myosin inhibitor management. His work has resulted in high rates of evidence-based therapy optimization, improved medication access, and streamlined multidisciplinary care. He is actively involved in outcomes research evaluating the impact of pharmacist-led cardiovascular care models.

Dr. Rajkumar serves as Chair of the Indiana ACC Cardiovascular Pharmacists Workgroup and is passionate about advancing the role of pharmacists in cardiovascular medicine through education, research, and collaborative practice. His professional interests include heart failure, preventive cardiology, obesity management, cardiometabolic disease, and implementation of emerging cardiovascular therapeutics.
